In compliance with your wishes you will find enclosed the form which you will please sign in three different places mark thus X You will please sign your name Knight and when <you> attend to this matter it is requisite to have one or two witnesses to attach their names under the printed word, "in presence of" to attest to your signatures, and when you have done this, enclose the whole in a letter directed to this Office, so that we can get Bro Knight's signature, and then <we> will return to you the Triplicate by due course of Mail.
